Who can get palliative and end-of-life care?

Palliative and end-of-life care is for anyone with a serious chronic disease or life-limiting illness. ​People often live for many years with 1 or more chronic diseases. Palliative care gives extra support at all stages of a chronic illness, not just near the end of life. Your palliative and end-of-life care team works with your family doctor and specialists as you're being treated for chronic illness. Palliative care provides many services to help you and your family live a more settled and comfortable life.

Living with a chronic illness can be harder some days than others. You may need to be in the hospital for a short time now and then. Sometimes medicines can help treat a chronic illness, and sometimes other therapies are a better choice. The palliative and end-of-life care team can help you look at your options. They wi​ll focus on your comfort, no matter what health problems your illness may cause. Your palliative care teams help you:

  • treat symptoms using medicines and other therapies
  • feel more positive
  • deal with fears and anxiety
  • make sure all caregivers know what you want and need
  • see what community supports are available to you
  • explore your care options and how the healthcare system works
