Heart failure: Weight chart

Heart Failure

Weight chart

Keeping track of your weight every day helps you and your healthcare provider know early if your body is holding extra fluid.

You can use the weight chart below to show any changes in your weight over time. Take this chart with you when you visit your healthcare provider.

To track your weight:

  • Weigh yourself every day—first thing in the morning after emptying your bladder (going pee) and before breakfast.
  • Wear the same type of clothing or no clothing every time you weigh yourself.
  • Make a note of your weight every day.
  • Call your nurse if you have a weight gain of 2 lb. (1 kg) or more overnight, 4lb. (2 kg) in 2 days, or more than 5lb. (3 kg) in 1 week.
