Registration is now completed entirely online. Beginning March 31, 2020 you do NOT need to sign and send a consent form. You may print a confirmation of your consent after you have registered and retain it for your own records.

The Alberta Organ and Tissue Donation Registry is the place to register your consent to become an organ and/or tissue donor.

It’s important that you understand what it means to be a donor and how important your choice is. Tell your family and loved ones that you ​have s​igned up to be a donor. They will then be prepared to speak on your behalf.
